What is Generic Accutane (Isotretinoin)?
Accutane is a form of vitamin A. It reduces the amount of oil released by oil glands in your skin, and helps your skin renew itself more quickly.
Accutane is used to treat severe nodular acne. It is usually given after other acne medicines or antibiotics have been tried without successful treatment of symptoms.
Accutane may also be used for purposes other than those listed in this medication guide.
What should I discuss with my doctor before taking Accutane?
Before taking Accutane (Isotretinoin), tell your doctor if you have
- a personal or family history of mental problems including depression, suicidal behavior, or psychosis (loss of contact with reality, hearing voices, or seeing things that are not there);
- diabetes;
- asthma;
- heart disease;
- osteoporosis (bone loss) or weak bones;
- anorexia nervosa;
- high cholesterol or triglyceride levels (types of fat) in the blood; or
- liver disease.
Do not take Accutane (Isotretinoin) if you are pregnant or if you could become pregnant during treatment or for one month after you stop taking Accutane (Isotretinoin). Accutane (Isotretinoin) is in the FDA pregnancy category X. This means that Accutane (Isotretinoin) is known to cause severe birth defects in an unborn baby. If you become pregnant, stop using birth control, or miss your menstrual period, immediately stop taking Accutane (Isotretinoin) and notify your doctor.
It is not known whether Accutane (Isotretinoin) passes into breast milk. Do not take Accutane (Isotretinoin) without first talking to your doctor if you are breast-feeding a baby.
How should I take Accutane?
Take Accutane two times a day with a meal, unless your doctor tells you otherwise. Swallow the capsule. Don't suck or chew it. Take it with a full glass of water, milk, or other nonalcoholic liquid. Follow your doctor's instructions carefully.
Depending on your reaction to Accutane, your doctor may need to adjust the dosage upward or downward. If you respond quickly and very well, your doctor may take you off Accutane even before the 15 or 20 weeks are up.
After you finish taking Accutane, there should be at least a 2-month "rest period" during which you are off the drug. This is because your acne may continue to get better even though you are no longer taking the medication. Once the 2 months are up, if your acne is still severe, your doctor may want to give you a second course of Accutane. If you are still growing, your doctor may recommend a longer "rest period."
Avoid consumption of alcoholic beverages.

What happens if I miss a dose?
Take the forgotten dose as soon as you remember. If it is almost time for your next dose, skip the one you missed and go back to your regular schedule. Do not take 2 doses at the same time.

What are the possible side effects of Accutane?
The most common side effects of isotretinoin are dry skin, itching, dry nose, nosebleeds (epistaxis), cracks in the corners of the mouth (chilitis), dry mouth, and inflammation of the whites of the eyes. Joint aches also are common. Patients may develop an increase in blood cholesterol and triglycerides. Psychiatric problems such as depression, hallucinations and suicidal behavior have been reported. Rare side effects include skin infections, peeling, sun sensitivity, hearing impairment and hepatitis. Rarely, isotretinoin can cause brain swelling (pseudotumor cerebri or intracranial hypertension), which produces nausea, vomiting, headache, and changes in vision.
What drug(s) may interact with Accutane?
While taking Accutane, do not take vitamin supplements containing vitamin A. Accutane and vitamin A are chemically related; taking them together is like taking an overdose of vitamin A.
The herb St. John's wort may interact with hormonal contraceptives (taken as part of iPLEDGE). Pregnancies have been reported by those patients who have used St. John's wort in combination with Accutane.
Remember, too, that Accutane should not be combined with tetracycline antibiotics such as doxycycline hyclate and minocycline.
